Measuring seated postural control in adults: a tool available in French (MCPAA 2.0) and Italian (SPCMA 2.0)

Background

Clinical measures of postural control when sitting in adults are not standardized, and most are derived from in-house tools. An initial study was carried out to adapt a pediatric instrument to assess this posture in adult wheelchair users:

Adaptation of a seated postural control measure for adult wheelchair users

The Measure of Seated Postural Control in Adults (MCPAA) is a very useful tool for postural assessment. Each section assesses: 1) the ability to remain and move while seated without support (1 item); 2) static alignment of the head, trunk, pelvis and lower limbs in the three planes (22 items), and 3) variations in postural alignment caused by an activity, propulsion or locomotor aid (22 items). Test scoring is based on an ordinal scale system.

The metrological qualities of this first version of the tool were evaluated, as well as its use in a clinical context.

Aims: To assess inter-rater and test-retest fidelity.

Method: Two expert occupational therapists assessed 33 adult subjects on two occasions.

Results: The test-retest reliability of section 1 was excellent (intra-class correlation (ICC)= 0.95), while that of sections 2 and 3 was moderate (0.60 and 0.62). Inter-rater reliability was rated as good to excellent for sections 1, 2 and 3 (ICC of 0.93, 0.68 and 0.69 respectively).

Benefits: Analysis of the results has led to the development of version 2.0, which has yet to be tested. However, its use is already improving the quality and consistency of assessments carried out from one assessor to another, ensuring more rigorous follow-up of clients over time and objective verification of the achievement of intervention objectives.
